Integrated Care Partnerships address strategic priorities

Integrated Care Partnerships have been working throughout the year to deliver improvements across a range of priority areas.

Drawing on the expertise, skills and resources across the ICP membership work is underway to address strategic priorities relating to diabetes, palliative and End of Life care, dementia, stroke, frailty, mental health and support to carers.

ICP committee members are also be using their learning and experience working in collaboration with LCGs and other stakeholders to inform the development of the Future Planning Model.

Health Minister Robin Swann approved a programme of work to develop a future approach for planning and managing health and social care services for Northern Ireland, based on an integrated care system (ICS) model.

The approach being taken will enable a system-wide focus on outcomes, driven by population need. Local providers and communities will come together to plan, manage and deliver care for their local population, with specialised and other relevant services planned, managed and delivered at a regional level.

This is a complex undertaking and a phased approach is being adopted to takeĀ  this work forward.

The new way of working will see us collaborating more closely than ever with a wide range of partners, both from within the HSC system and beyond. It will harness the strengths of different parts of the system, across traditional organisational boundaries and across sectors, to deliver improved outcomes for the people of Northern Ireland.
