General Practice Health Visitors and District Nurses
Health Visitors and District Nurses are part of the multi-disciplinary team (MDT) within a GP practice.
District Nurses
District Nurses in a GP practice work with any adult who has an identified healthcare need requiring nursing support. Patients can be referred to the District Nursing Service if they require a nursing assessment and meet one or more of the following criteria:

Unable to leave home due to mental or physical illness.

Have a nursing need which makes a home visit more appropriate.

Have been identified as having specific nursing equipment prescribed for home use.

Have been identified as palliative or require assessment of end of life care needs.
Health Visitors
The Health Visiting service is a universal home visiting service which delivers the child health promotion programme for Northern Ireland.
Health visitors are registered Nurses / Midwives who have additional training in Specialist Community Public Health Nursing.
The service delivers a programme of care to promote and protect the physical, emotional and social wellbeing of individuals, families and communities through a family centred approach for all preschool children.
